My name is Habon Mohamed I am a dual certified Psychiatric-Mental Health Nurse Practitioner-Board Certified (PMHNP-BC) and Family Nurse Practitioner-Board Certified (FNP-BC). I specialize in assessing, diagnosing, and treating individuals with mental health disorders. With a keen understanding of psychiatric medications and therapeutic interventions and evidence-based practices, I am committed to providing patient-centered care to individuals facing mental health challenges. I work with individuals with depression, anxiety, life stress, schizophrenia, developmental delays, BIPOC, LGTBQIA+ and those wanting change from substance abuse. I strive to work with clients sharing my experience, scientific research, and their goal to reach their highest potential. I have experience working with ages 16+ in areas of electroconvulsive therapy (ECT), inpatient and outpatient co-occurring mental and substance use programs, serious and persistent mental illness, developmental delays/ASD, MAT (suboxone/vivitrol/naltrexone) and more.

In my practice, we are currently accepting NEW patients for medication management, therapy, chemical dependency treatment including suboxone. We also provide esketamine/spravato. My interests are varied and include a wide spectrum of disorders, such as major depression, anxiety, bipolar, schizophrenia, substance use, ADHD and PTSD. I explore the biological, psychological and social components of mental illness and utilize tools and interventions to evaluate and treat my patients, including diagnostic laboratory tests, prescribed medications and coping strategies. We accept most insurances and private pay.

Otherwise we will use counseling techniques such as CBT and DBT to look at the patterns of thought that may be holding you back.

As a nurse practitioner certified in Psychiatry, I believe in providing each patient with holistic, comprehensive care that is effective, efficient, and compassionate. I also believe in a team approach where I collaborate with the patient and other providers to determine a plan that will bring lasting wellness. I am board-certified by the American Nurses Credentialing Center and have a special interest and expertise in treating major depression, anxiety, ADHD, and narcotic addiction.

I am board certified to treat children, adolescents, and adults for simple, multiplex, and the most challenging psychiatric disorders. I have extensive experience with hospital, community, and outpatient practices with a substantial range of patients from different ethnic, sexual, and socio-economic backgrounds. My patients often say that my distinctive perspective helps to form the open, trusting, and personable relationship I have with them. As a prescriber of psychiatric medications, I enjoy working with a wide variety of patients, and digging deep to find the solution(s) to the difficulties that might be causing you distress.
